Flowers Functional Medicine, PLLC. is a fee-for-service medical office. We do not accept private health insurance, and unless otherwise agreed in writing, payment for medical services or any laboratory fees is expected in full at the time of your appointment. Payment can be made in the form of credit card payment only. If you have a Phone or Virtual appointment with our provider, the most recent credit card that we have on file will be charged to take care of your payment. Flowers Functional Medicine is committed to practicing medicine in a professional and financially responsible manner. We realize that there are situations that may affect the timing of your payment. In such cases, requests for payment plan arrangements may be made by contacting BEFORE your visit. Keep in mind, however, that it is your responsibility to contact our office to make such arrangement requests, and it is not a guarantee that a payment plan will result. Payment plans may not be initiated with new patient visits.
