The intake process begins by sharing your health, your symptoms, and your personal story. I want to know your goals and concerns so we can create a firm foundation on which to build a better you.
This process includes:
Health history questionnaires: This comprehensive document includes targeted questions that will begin to untangle the root causes of your symptoms by taking a deep dive into your complete health history. It can help us discover if there are any potential triggers or factors that could have led to or contributed to your health concerns.
Initial appointment: After your health history paperwork is complete and intake fee is paid, we will discuss your specific health challenges during a 90-minute appointment. We will talk about your symptoms, goals, and concerns, as well as any preliminary interventions, lifestyle changes, supplements, and treatment recommendations that can kickstart your healing journey before our follow-up appointment. I will also order a comprehensive baseline functional blood panel to be completed before our follow-up appointment.
Comprehensive baseline functional blood panel: A simple blood test will be shipped to your house complete with directions to a local lab so you can have your blood drawn. These blood tests go deeper than a traditional health checkup. A single blood test can provide us with essential information about your hormones, thyroid, vitamin markers, metabolic markers, inflammatory markers, and more.
Additional labs and tests: Based on the information you provide, additional labs, imaging, and tests may be recommended. Only labs and tests you actually need based on your unique symptoms will be ordered to keep costs down.
Based on the information you provided during our comprehensive intake appointment, I will conduct an extensive, in-depth review of your health history and lab reports through the lens of both conventional medicine and functional medicine. I will rule-out any urgent/emergent problems to ensure no serious medical problems are overlooked that may require more conventional methods of treatment.
Based on the information uncovered, I will outline an easy-to-follow plan that includes interventions, lifestyle changes including specific diets , supplements, and/or treatment recommendations that are specifically chosen to help you tackle your unique health problems and reach your wellness goals.
We will evaluate your symptoms, after preliminary interventions, and do an in-depth review of your lab results. In this 60-minute appointment we will begin to unravel the true cause of your symptoms.
Based on this information, we will develop a plan, that is based on your readiness and ability, with consideration of your lifestyle, finances and life circumstances. During your follow-up appointment, you will be able to ask questions so you can truly understand how to transform your life with the sustainable improvements outlined in your plan. In addition, we will talk about the approximate number of follow-up appointments you will likely need, the approximate time it will take for you to recover and feel better, and any other ongoing treatment options you may want to consider. Keep in mind wellness is a journey and this process can take between 6 weeks-6 months.
Partner with a board-certified nurse practitioner who is able to diagnose and treat medical conditions, using both a conventional and functional approach.
Labs can be directly evaluated and diagnoses made to create a treatment plan based on those labs.
Receive prescriptions directly from Kristin Flowers, FNP-C that can be covered through insurance, depending on your coverage.
Because you’re receiving treatment from a medical provider, scheduled doctor’s visits, lab work, and some supplements may be covered by your insurance or an HSA.
Bioidentical hormones prescriptions, if they are part of your wellness plan.
Referrals to specialists, when necessary, with consultations with those specialists informing the details of your wellness plan.
